Transforming Care is about making health and care services better for people with learning disabilities who have mental health conditions or behaviour which challenges.
The National Forum for People with Learning Disabilities and the National Valuing Families Forum have been asked to find out what carers and people with learning disabilities know about transforming care. Please fill in this Easy Read questionnaire to help us find out what people know in the East Midlands.
Once we have collected this information it will be looked at by the Regional Co-ordinators and the National Forum. Inclusion North will then collect all of the information from across the country and send it on to NHS England.
